Insurance questions

We accept many major dental insurance plans, including Delta, Viva, and UnitedHealthcare. Coverage and network status vary by plan, so call before your visit with the name of your insurance company and plan.

Insurance information or a benefit estimate is not a guarantee of payment. Your insurance company determines what the plan covers and what portion may remain your responsibility.

Forms and dental records

This site does not currently provide downloadable patient forms. Call the office if you have a question about paperwork for your visit.

If dental records or X-rays may need to be transferred, call before arranging the transfer so the team can tell you where they should be sent.

Most new-patient appointments take about 75 to 90 minutes. Call with questions about scheduling, rescheduling, or an upcoming visit.
